To make sure that your student receives appropriate care in case of emergencies, we ask that every parent update their child's clinic information. This information will be used to treat your child and contact you if an emergency occurs involving your child.

This information needs to be completed in its entirety and signed by the parent. Students must EITHER bring a completed clinic card with them for schedule pick up OR have a parent present to complete the form on the day of schedule pick up.
