All Band and Color Guard members need a current physical to march 

Please have these completed before band camp starts. All physicals must be on the Mill Creek Form - see attachment

Band Expense Reimbursement Credit Form

Parents/Band Students

Please use the attached Expense Reimbursement Credit Form when a reimbursement or a credit needs to be issued to a student’s account.  This form will enable us to perform our accounting tasks much more efficiently.  Thank you for your support!
 

SCRIP Fundraising

Visit the Fundraiser page for an explanation of the SCRIP Fundraising Program.  You can download an order form as often as you need, fill it out, bring it to the school lobby on Tuesday between 5:00 and 6:00pm, along with your payment, and your scrip will be available for pick-up the following Tuesday.  It's that easy!
