Physical, Insurance, Consent, Concussion Management Form
In order to participate in any athletic activity at Mill Creek - camps, pre-season conditioning, weight trainings, try-outs -
A current physical, along with insurance information, and parent consent must be on file in the athletic office prior to beginning any activity. All are included in our MC Physical Form.
AlSO, due to the passing of a recent state law - Georgia Return To Play Act - we are now mandated to have all parents receive an information sheet about concussion management and this form must be signed by student/athlete and parent. This form should be returned with the physical form.
Only Mill Creek forms will be accepted and can be downloaded from the web site or picked up in the main office of the school. Please be sure to print all 4 pages of this form. The last page must accompany the actual physical form. Also, the Concussion Management form must be turned in with the physical.
