For Students in Grades 9 thru 12:
In order to participate in any athletic activity at Mill Creek - camps, pre-season conditioning, weight trainings, try-outs -
A current physical, along with insurance information and parent consent form, must be on file in the athletic office prior to beginning any activity.
Only Mill Creek forms will be accepted and can be downloaded from the web site or picked up in the main office of the school. Please be sure to print all 4 pages of this form. The last page must accompany the actual physical form.
